How to Install CyanogenMod 12.1 on ASUS Zenfone 4 :
Make sure your Zenfone 4 firmware is the latest 7.3.3
Unlock Bootloader, Unlock tool can be download here

Extract Unlock bootloader tool > install apk> unlock
Download CyanogenMod 12.1 ROM Here

Download ADB tools if you don't have it
Download Link: [Please login or register to view this link]

Download Intel usb driver if you don't have/install it
Download Link: [Please login or register to view this link]

Copy boot.img, system.img and recovery_TWRP.img place it to ADB folder
Turn off Zenfone 4
Enter Fastboot mode (Power + Volume up)
Connect Zenfone 4 to PC
At ADB Folder press shift+right click > open Command here
At command prompt type (press enter after each line and wait until finish) :
fastboot flash system system.img
fastboot flash boot boot.img
fastboot flash recovery recovery_TWRP.img
Factory Reset
Done
